Simply click the 'here' to register and you will see the following screen
Fill in all the required fields and any of the optional ones (indicated with *) that you wish to include. Note that we are asking for a program name. This applies to currently active residents and the purpose is to allow the program directors to view resident's progress through the course (only if you explicitly consent to have them do this at the bottom of the form). The email address is important so that we can send your password to you if you forget it. |
